Descriptions (the description of the data to include in
the excel file. Please refer to the VALUES column for
details of the data type as well as the required values.)
Supporting Documentation – Please
provide
all documents in PDF format. Each claimant should have
a separate PDF file for different document types (Certified Claim
Form (must be included), Exposure, Medical, PR/Estate).
The name for each PDF file should contain the SSN of the
claimant.
